Please complete these forms at home and bring them with you at the time of your appointment. Please remember to bring all pertinent imaging studies (X-Rays, MRIs, Bone scans) and reports with you, as well as your valid insurance information and form of identification.
All appointments are confirmed 1-2 days prior to the scheduled date. Kindly give the office at least one business day's advance notice if you cannot keep your appointment.
In case of an emergency that occurs after hours, on a weekend or a holiday please call the main office telephone number, press 0 and an operator will assist you in contacting Dr. Meere. Alternatively you may consider going to your local emergency room, the immediate care unit at the HJD or the emergency room at NYUMC.
Every message is important to the office. However not all messages are urgent. Dr. Meere's staff is trained to prioritize phone calls. Non-urgent messages left on the voicemail will be returned within 24 hours. All pertinent medical queries are discussed with Dr. Meere. In order to avoid delays in response time, Dr. Meere may delegate the answer of routine issues to his staff. Whenever personal communication with the patient is necessary Dr. Meere will personally call back.

Please call the office to request the above. The response time is set at three business days from the date of the request. Physiotherapy refills can only be issued once an updated progress report by the treating physiotherapist has reached the office.
Please call the office to determine insurance coverage and participation in a particular health plan. At the time of your appointment please bring your insurance card and a valid form of identification (driver's license, passport). For your convenience we accept cash, check, Visa, Mastercard, American Express and debit cards.
If your insurance coverage is through Worker's Compensation (WC) due to a work-related injury, you will be asked to provide the office with the following information: name, address, and the telephone number of the WC insurance carrier. In addition the date of the accident and the open WC claim case number will be required. Upon verification of this information we will call you to schedule an appointment. If the claim is denied by the WC insurance carrier, despite verification, the patient will be held responsible for reimbursement of services rendered.
If your insurance coverage is through a no fault (NF) insurance carrier due to a motor vehicle or other accident, you will be asked to provide the office with the following information: name, address and telephone number of the NF insurance carrier. In addition the date of the accident and the NF insurance carrier claim number will be required. Upon verification of this information the office will call you to schedule an appointment. If the claim is denied by the NF insurance carrier, despite verification, the patient will be held responsible for reimbursement of services rendered.
