Please print and complete the forms below prior to your appointment and bring them with you to the office. This will facilitate the registration process at the time of your visit:
Please remember to bring all pertinent clinical information with you as well. This includes any imaging studies, i.e. X-Rays, MRIs, CT-scans, and Bone Scans, and laboratory reports. If you have had surgery previously please bring your operative reports and arthroscopic photos.

Appointments are confirmed 1-2 business days prior to the scheduled date. Kindly give the office at least 24 hours notice if you cannot keep the appointment.
In case of medical emergencies please call 911. In case of overnight, weekend or holiday orthopaedic emergencies please call the office at (212) 263-2366, press 0 and an operator will assist you in contacting Dr. Meere. You may also try to reach Dr. Meere by calling the office cell phone at (347) 403-1006. Alternatively, you may consider a visit to your local emergency room, the Immediate Care Unit at the NYU-Hospital for Joint Diseases at 301 E. 17th Street, NY, NY or the NYU Langone Medical Center Urgent Care Unit.

At the time of your appointment please bring your insurance cards and a valid form of identification (driver’s license, passport). For your convenience we accept cash, check, Visa, Mastercard, American Express and debit cards.
